Question about nightshift Tele preceptorship

Hi all,

I'm about to start a night shift Tele preceptorship.... and I'm pretty curious as to what it will be like.

Whats the ratio? What kind of things should I bone up on before I start?

Our tele ratio is 4:1. Brush up on your cardiac meds, rhythms, and all the usual med-surg common probs.

Our tele unit is 4:1 I agree with the above poster, brush up on your rhythms, meds, common problems. Also brush up on the units protocols, especially drips and which one(s) cause patients to be transferred to a higher unit within your facility.

Do you think that a night shift is still a good learning experience? Should I work on switching to a day or at least evening shift?

Nights might be a better experience for you, actually. Many cardiac meds are TID/BID, so you will get experience passing meds. Most IV's will run around the clock. In my experience, just as many (if not more) clinical issues happen at night. In your down time you can take a better look at the patient's history and talk patho with your preceptor. The only things you are missing out on are morning baths and discharges.

BTW on my unit our ratio is 5:1 at night, 6:1 on minimum staffing, but that is rare these days :)

Hmmm thanks.

What about placing IVs? does that happen on night shift?

Our new admits on the night shift come up from the ED with IVs in place. There are always IVs that need to be replaced at all hours of the day and night.

The ratio will be entirley up to your facility. My experience is 6:1.

Learn the polices and procedures.. most importantly .. be aware of any standing orders you can and should implement.

Treat the patient.. not the monitor. If you have concerns about your IV skills make sure you get a day with the IV nurse in orientation and go to the expert on duty for any and all IV placement .. learn from the best!

Def will keep you all posted. Trying to switch to an ED post, but I think my chances are slim.
