Seriously?
I read all the definitions....near miss all the way to sentinel event. None of them fit.
This seems silly to me.....

I will chart that the pt removed the tube, but I refuse to chart an incident report. EVERYTHING is an incident that happens that isnt supposed to.

I would definitely write up an incident report just in case the patient injured their self and it wasn't evident until later or if the patient wants to sue for whatever reason. It isn't an "event" per say. Then investigate if the NG tube is really needed...if it is we usually just put another in.
