Mr. RD, a 72-year-old female, resident of Terrace Lodge was found lying on the floor in her room, with a puddle of fluid beside her bed at around 1430 while doing hourly checks to the resident.

He sustained a visible contusion on his forehead and skin tear on his right elbow. Vital signs were taken and assessed level of consciousness. The puddle of fluid was immediately wiped off the floor and continued assessing the resident. Checked for his leg rotation and there is no sign of hip, pelvic or spinal pain. However, there is tenderness noted on his right leg due to the fall.

The nursing supervisor was notified after the incident and resident was assisted to get back to bed. Bed brakes are on and call bell is within reach. Upper rails of the bed were raised to ensure safety. Will continue to monitor the patient with neurologic and vital signs check every hour for the first four hours after the incident. -----------------------------------------------------migueros,RN

Add:

1. found by who? Migueros, RN or Shelly, CNA?

2. Get rid of the word "around", say "approximately" instead.

3. Is this a him or her? Your first paragraph switches back and forth.

4. "He sustained a visible..." should not be there. You don't know if the fall did it or not because you didn't see the fall. You can say "Contusion noted to forehead" instead.

5. Who checked the pt for leg rotation, etc? State a name here.

All in all, a good attempt.

I will note that we see a BUTTLOAD of students coming here and asking us for answers to their homework instead of help with their homework. Posting your work (like you did) is how we can see that you are looking for help rather than someone else to just give you the right answer. Being angry at people who you are asking for a favor from is the wrong way to go about it, ok? Always make sure you follow up some help with a "Thank you."
