my not so lovely night

so last night was a disaster heres what happened...

My unit partner called off (lol everytime she calls off or goes on vacation something happens) so I had a PRN nurse with me ok not that big of a deal. Then I get a unanounced admission during report (yippie skippie). A code was called on someone who was sleeping yay adreniline! Get through my first med pass and am working on the admission papers just as i get all the meds written out on of the aids comes up in the middle of dinner for the residents and askes if she can go home (with big ol **** grin on her face) I ask her why she says she doesnt feel good (still with a **** grin on her face) and im sitting there thinking and you havent even been on your assignment half the night cause your wandering around the other units. So i said to her 'so you want to go home cause you dont feel good but yet you have a big ol **** grin on your face?' her answer 'yeah' then I told her to go talk to the shift supervisor. after she left the floor to talk to the supervisor another aid came up and complined that the other aid (the one that wanted to go home) smelled like ETOH, just freaking wonderful! I call the supervisor and ask if she sent her home, she said yes and i explained what i was just informed of that aid told the supervisor that I said it was ok for her to go and that she reported off to the other aids and that the assignment was split up, well that was a huge lie! NONE of the other aids even knew she left! Well girlfriend got herself a write up and automatic termination for abandonment (i guess she was telling people it was her birthday)....well happy birthday heres your pink slip.

then i go down to our med machine to see if my admits meds were ready and they were not so back to the floor to start my second med pass I get part of my hall done and the next thing i know an aid is flagging me down to come look at something, i walk in the room put on gloves walk around the curtain and blood everywhere! HOLY CRAP! in report i was told this person was having pink, PINK urine and a ua/c+s was ordered, um yeah ok. this person was soaked in bloody urine like gown underwear and pad (the pad can hold 1500cc) and not only was it bloody it was like jelly like someone dropped a jar of smuckers in this persons lap! I called the supervisor and she came we got this person cleaned up and she examined her, then i paged the md many many many times... NO answer! OMG freak out moment! then we call the DON they dont answer, then the ADMIN for the facility they say to call the medical director call the medical director they give me three orders 1. call primary md q2 hrs until reached then d/c 2. get better # for primary 3. send to hosp if poa agrees and if not do labs in am. Nothing like being ordered to blow up a pager. anyways POA denies transfer, will carry on with labs in am. then about 20 min after that the primay calls back and informs me that this person has a do not hospitalize order (thats nice cause it was the first thing i looked for and couldnt find) grr and i never did get a better number for him.

ok finish my second med pass late try to complete the admission the pt refused to let me do a complete skin check, double grr. charted my butt off for two people. then i had my pt/inrs to do and clean up what the previous shift didnt finish and help my unit partner with an iv. I had no break no food no drink and forget about going pee i had to stay an hour and 20 min over and the next shift supervisor was nit picking stupid stuff that she could do her self and the nurse who took report from me would not help for anything i told her about my crazy night then she told me 'well you need to blah blah blah' i didnt care anymore i haad it she was barking stuff at me while i was was trying to chart and ended up writing what she was saying. oh and to top it off i a pleasently confused person who was up out their w/c trying to put people to bed and a new person who was trying to kick out the room mate cause they thought that their oldd room mate was going to move in there too.

lol so that was my lovely night, my only question is whould you have written the order to call md q 2 hour and get a better number for them in the pt's chart or just take it as a verbal order for the nurse to do outside of the pt's chart? cause next shift supervisor wanted me to write a t.o. for it and i told her that, that would be stupid cause it throws the md under the bus and makes it look like we dont know what we are doing.

I dunno how I would have written that order, but a bunch of big, squishy hugs.

I didn't know you needed an order to call the MD and get a better contact number??

Leave it out of the chart. It doesn't have anything to do with patient care and since you reached the MD the order is moot anyway. However, I would definately include in my note who you called, the time you called, and what the response was- including every time you called the MD with no answer.

Sorry you had such a horrible night. It sounds like you handled it exceptionally well. Here's to hoping you don't have a other shift like that for a long time.

I am so sorry that you had a terrible night, just reading that made me feel so bad for you! I swear when it rains, it pours! I hope the rest of your week goes much more smoothly! :hug: :)

What a horrible shift. Sounds a bit like LTC? BTDT and have a t shirt.

I wouldn't have written it as an order..maybe in the notes? Eh.

uh.....hug? Drink? Nerf balls to throw until you feel better? (that last one actually works for me)

I'm not in LTC (although when we have people in ICU for months I sometimes feel like I am...), but I've had shifts where Murphy stomped in and set up shop in the nurse's station and everything just...went....wrong.
