good report sheet

    Does anyone have a really good report sheet for night nurses? I don't need one with a lot of the blank assessment info on it really. Well...I guess I don't really know what I need on it lol. Please feel free to email me any that you have available,,,thanks so much! sweettls32@yahoo.com

    Have you searched allnurses. Search for report sheet or brain sheet and a few will come up.

    Its a start.

    Also, the Scrubs magazine website has a few good Report sheets for ICU nurses.

    ICU report is usually pretty basic because the nurses are generally type-A personality, no matter what you tell them they will double check anyways. I usually cover the background then go head to toe.

    -Admitted for:
    -MD:
    -HX:
    -All:
    -Neuro:
    -Cardiac:
    -Respiratory:
    -IV/IV Fluids/Drips:
    -GI:
    -GU:
    -Skin:
    -Labs:
    -Other Miscellaneous Data:

    -Pretty basic, but you know you've got a shaky ICU nurse when they give you a 25 minute report.

    Or they are like me, and just love to run their mouth about unimportant stuff. Which now that I think of it, isn't technically part of report...


    On a serious note, I am def a type-A when it comes to report. I just use a blank peice of paper and only write a few quick notes. I trust keeping it in my head more than on a piece of paper I might lose...

    Great report! This is how I prefer to give report, and receive it. Very concise!
