Speaking as the former compliance officer, I would report it to the facility compliance or privacy officer.

Is this something you witnessed as a student in a clinical setting, or is this an assignment you are working on? Knowing the reason why you are asking this question would be helpful in giving you a response. If it is something that you saw in the clinical setting let your clinical instructor know your concern.
It's a job interview question, and I'm not sure if there's a set standard for how to go about addressing it.

There is if witnessed follow company reporting protocol usually compliance officer
Speaking as the person who does interviews, I would give full marks to a candidate who would report it to the privacy/compliance officer. Reporting it to your supervisor would also be acceptable.

OP is saying that she was asked this question during a job interview, not that she feels a HIPAA violation occurred in a job interview.
