I read in the Saunders NCLEX-PN book that two answers alike are generally not going to be the correct answer, but then I read in Lippincott (I believe) that you should narrow it down to the two most similar answers. I'm confused can someone please clarify which test taking strategy works best. Thanks.

The wording for both is slightly different of your examples.
This is what I was taught. If two answers are the same but in different words such as a) breathing fast and b) tachypnea, then they are both wrong.
Also, if you have two answers that are the opposite such as a) bradycardia and b) tachycardia, then one of them is likely to be correct. However, some answer have all four answers being opposite to each other, so that it doesn't help at all.
