What would you do when you have incomplete medical records for a patient?[TABLE=width: 90%]

[TD] Ask a supervisor what to do[/TD]
[TD] Ask the patient what to do[/TD]
[TD] Look at what has been done in the past in similar situations[/TD]
[TD] Wait until someone tells me what to do[/TD]

So it's obviously not asking the patient! LoL, but which of the options would you say is the best option in this kind of situation?

pt safety, its really not enough information...how is the diagnosis, you have to treat the pt to your best ability then think of records. pt first, machines and papers later...my reasoning
