The best care plan book... is the one you can toss in the fire after 2-4 years of h***.
To be honest, I used several.
When I started nursing school, I had recently graduated friends who could not toss me those care-plan books fast enough... like keeping them in their possession for even a nano-second longer than they had to was like acid on their very soul... but I digress...
Actually, I found all books equally helpful and used Google to find other sources and examples.
However, when you go online, be warned that some care-plans are not good examples of what you want. But then, I figured if I could tell it was a bad care-plan and what was wrong with it, well then, I guess I had learned something somewhere!
My favorite was the Davis pocket-guide. They also have a website.
Use many resources for a wide variety of fun.
Fun.
Cheers.

The book I highly recommend to those I mentor is Mosby's Guide to Nursing Diagnosis.
I find this book very helpful for several reasons:
One: Small size and easy portability.
Two: Mosby is a tried and true source. As with all things research wise, beware the internet. Not to say you can't use it.....just be smart about it.
Three: Breaks down Nursing Diagnosis based upon Medical Diagnosis. Takes the guess work out of associating the two.
Other books I tend to favor:
Nurse's Pocket Guide: Diagnoses, Interventions, and Rationales
Nursing Diagnosis Handbook: An Evidence-based Guide to Planning Care
Finding a careplan book is a lot like trying on jeans in the dark: You know its a good fit because it just feels right.
Best wishes.
