I had a pt last night w/ great BP's, occasionally a 140's/80's. No s/s Pre-E. Uric acid was 5, LDH was slightly elevated, Creatinine was 0.9, all other labs were fine. The results of her 24hr urine had 10,790mg of Protein/ 24 hrs!!!
10 Grams of Protein?? I actually called the lab b/c I didn't think that was possible. When I called the doc, she said it doesn't happen often, but it can happen. She was thinking some type of atypical Pre-E or kidney process going on.
So what's the most you've seen in a 24 hr urine?

I'm trying to think back to when I worked in the lab and used to run 24 hour urine proteins often. I'm sure I've seen them that high before. If you're around long enough, you see everything.
High levels like this could also happen with Bence-Jones proteins from a multiple myeloma patient....

My own hit 14,000 when I had HELLP Syndrome.

I think the highest I've seen (that I can remember, anyway) was somewhere around 4500-5000 mg.
He's great! Thanks for asking. He just turned 10 this summer and aside from being in the 7th percentile, has no permanent issues from prematurity. It took a year for my kidneys to straighten out and another 6 months for my BP to come down, but I eventually recovered completely.
How far along were you when this happened? We've had a few HELLP patients where I work, and I know of two other women who had it, and NONE of the babies did well, even the ones at term. One of the women died, followed by her baby 3 months later, ostensibly of SIDS but I think it was the aftereffects of his tragic birth because he was very sickly despite being full term. The other mother survived the birth but her 24-weeker died after IMHO being cruelly tortured for three months in the NICU.

We recently had a patient who's 24 hour urine was 15,000.
No kidding. We thought it had to be a mistake, but it wasn't.
She had mild high BP but no other signs of PIH or HELLP.
We induced her anyway. Probably an underlying problem with the kidneys.
Pre-eclampsia developed at 26 weeks, HELLP at 28 and delivery at 29. In the week before delivery, I had two other 24 hour urine tests that came back at 4,000 and 11,000. My son spent 11 weeks in the NICU.
We had a 24 weeker in our NICU as well. She got released after 9 months. Her poor family really struggled.
I really respect all the nurses that choose to work in this field. I couldn't do it, so my hat is off to all of you that can and do. You bless the lives of so many people.

Coming out of lurkdom - mine was 5250mg at 35 weeks. Everything was ok and I was delivered immediately. Severe preeclampsia.

There was a lady I remember who had 6grams. Too bad she was only 21 weeks
