In addition to wearing a scrub jacket, can anyone recommend a way to stay warm? Is there a special warm undershirt that I can wear under my scrubs that will not be visible?

How about some kind of Under Armour garment? You can get them at sporting goods stores. I also have the opposite problem. I never stop moving so I'm always hot. I also find wearing a scrub jacket a hinderance. Always feel like the sleeves or sides are going to contaminate something.

I wear the cuddl duds(spl correct) long underwear. They are soft inside yet a thinner material designed to wick moisture away from your skin and great for the colder areas you work in. They have tops that you can get in tank, long sleeve and short sleeve. They have several different colors and you can find them at JCP, Wal-mart and online. When I bought my first pair, I thought that they were too thin to do anything but they worked great for me.

I use the tiny tees from Old Navy. I wear them year round under my scrubs as I find jackets and sterile fields do not go together very well.
