I'm a registered nurse from the Philippines and I'm planning to apply for visa screen. I'm rostered to take the CGFNS qualifying exam this November. It says in the application form for visa screen that I have to list all the schools i attended for my nursing education. My dilemna is, I'm a second courser, my nursing school incorporated my transcipts from my previous course which is a health related course in my transcipt for nursing. CGFNS accepted the transcript from my nursing school. Do I need to get a separate transcript from my previous course and a separate license verification for my other license and give it to ICHP in order to have my visa screen?

In my opinion, you don't need to submit the TOR from the other course unless you are specifically ask to. And you don't need to submit the other license from your first course since it is not nursing related. Only nurse's license will do.

Depends on what was initially submitted on your behalf for the exam, if it was the set of transcripts, then there is nothing else that you need to submit. If your school only completed that one page form with sepcific hours, then you will need the transcripts.
If the full set was submitted for the CGFNS exam, then you only need the application and fee, and do not need to resubmit anything.
Make sure that what ever you put on the application for the CGFNS exam is what you put on the application for the VSC, or you will spend months trying to straighten things out. All information should be the same, down to address even.
My school copied in verbatim my transcript from my previous course and they just added the nursing subjects i took to obtain my BSN. This was the same transcript that my school provided when I applied for the CGFNS qualifying exam and with my NCLEX application. I called CGFNS and I was informed that I'm rostered to take the test this November. So it's safe to assume that I wont be getting my transcript from my previous course since CGFNS accepted my transcript?
Then you are all set. The issue that comes up is when the school doesn't actually send the transcript but just fills in that one page form. You can qualify for the exam with just the one page.
This is why it is always important to make sure that the school also includes the transcript.
My school included my transcript when I applied for CGFNS. I was the one who closed the envelope and I checked the contents of the envelop thrice before closing it.

i have the same question as with philrn, i'm a second courser of nursing and my first course is also a health related which is a medical technologist...i'm planning to pay for visa screen tomorrow, tho my nursing transcript is already incorporated with my transcript from my first course....do i have to go to my first course school and ask for another transcript to send to ichp or just send my nursing transcript??? and do i have to send another verification of my license for medical technologist or just the nursing license? i've sent my forms to my school of nursing and for them to send it to ichp..from what i have read in the visa screen checklist aside from listing all the schools whether you have completed or not, i have to mail a request for academic records forms and transcript...the same goes for licensing authorities whatever license i have held, whether current or not, inactive or expired..it is stated that i have to mail request for validation of registration/license....am i doing the right thing??? just wanna know...i'll be taking my nclex on november...hope to clear my mind from it :)
Just make sure that your schooling is listed exactly has it has been on the other forms.
..i have a 5 pages of my transcript of nursing incorporated with my first course...how about the licensing agency? i'm going to ask them to mail 2 validations, one for nursing and the other for med-tech, right?
