In OR , they use vancomycin to wash wound and what is the difference from vancomycin IV

None, at least in my facility. Same solution, just used as irrigation instead of intravenously. Why do you ask?

We're even using it in my facility for a study in powder form with ortho implants. Same stuff.

Used it yesterday to soak a bone graft before being placed and then sprinkled in the site before closure
wrong order of Vancomycin was placed by MD, so pharmacy sent mixed in 800ML bag for wound irrigation, but the right order was suppose to mix in 500ML IV. The concentration and site was wrong ,but in case of emergency and rush, if it was given to the patient by IV site. The antibiotic will cause harm or it only has less effectiveness to the pt as it was mixed in 800ml.
Did they get the same dosage of Vanc.? If it was the some dosage there should be no problem, just a little more diluted.
