Our c-secton rate in the USA is out of control. I am glad to see the ACOG discussing TOLAC/VBAC for the truly viable option it is for the majority of women. And I am glad to work in a facility that "allows" TOLAC/VBAC. More need to get on board and we need to educate our patients about this very important option.

For those that are currently doing VBAC's regularly...Does your OB stay in-house? Asleep in a call room? What about during office hours?

The only rupture I ever saw was in a repeat c-sec. Tiny window with a little finger poking out. Both were fine. She was a 4 timer though. I think the OB strongly rec. no more pregnancies for her. Im so glad ACOG finally got on the boat. On c-sec rate is the craziest thing I've ever seen in the last few years! Wonder how long it will take for practice to catch up?

I'm in a teaching hospital, so residents and attendings are always there no matter what. If the private docs have actively laboring pts they are in-house as well. That goes for nights. They are almost always there during the day, not at the office seeing patients. I don't think they schedule pts for whoever is on-call.

Once active labor starts the OB stays in house. The birth can not be induced in any form. Their is a limit on time between TOLAC and previous C/S and # of C/S; but most qualify.

And just to point out. It is more dangerous to have a repeat C/S than a TOLAC.
