V codes/field staff

I was curious how many home health agencies have the field staff code their OASIS or if you have someone internal do it?

Thanks,

they don't want us to do it:the QA nurse does this.I interviewed and did a brief orientation with another hh company (that I ended up leaving right away)that had a seperate form the field nurse filled out and gave to the office for this purpose. It gave the field nurse input and the QA nurse the final say.I thought this was a good idea.I wish the place I am at now would have some type of organized chaos for this problem.

At my agency each team has a nurse that does the coding. She also figures out the number of visits that are allowed. I find this really helpful when developing a care plan. I'm not limited to that exact number of visits, if the patient clearly needs the care we deliver, but I do try not to go over the allowed number of visits.

Our field nurses identify primary and secondary diagnoses, coding is done by our QA RN who computer enteries info---over 800 admissions month! Case conference held if a differing diagnosis is recomended---or something missed.

And of course when she's on vacation---my Intake RN's and I code charts and review OASIS :)

I am a case manager/admitting nurse & I do my own coding, including the new V-codes. But if I have questions I call someone a little higher up than me.
