question on Cerner ,example. I came on a shift, old nurse is giving me report, there is an issue & she's taking care of it when I come back to do my "assessment" & my "ongoing assessment"
she has the assessment opened on her PC because she is looking at stuff while on phone with another dept etc.
THEN I open the assessment, add stuff, update it and I move on to the next pt. she THEN adds whatever she's adding, does her update (with the info that has BEEN on her screen for 10 minutes PRIOR to my update)
DOES SHE THEN WIPE OUT WHAT I'VE DONE? Because her computer screen has the info from before i did my assessment?
isn't it over riding what I did?
I ask because a few times I feel like, ya I know I did that already, but it's not in the computer! I kind of doubt I forgot to hit "update" because I do it so often out of habit!

We use cerner too and as far as I know two people can chart on the same pt at the same time. I'm sure it happens with the NAs putting in the VS and my charting assessments. It should save everything. If by chance two things were charted at the exact same time, it will document both.
Say for instance, two BPs were put in for 2000, then the chart would loll like 120/80 [2] and if you click or hover over it, it will show both.
At least that's how our system is set up.

the program is created to allow more than one person to access the record at the same time. Data is in real time, so it does not matter when the program was opened. That means if, at 0900, you chart that the patient voided at 0830 the program will indicate the time voided, but will also keep record of when you recorded that value. Designed to allow people to chart after the event, not during it. Will not let you chart ahead of course.

I used Cerner when I worked in the hospital and there is some way to change your settings so that it will allow you to chart ahead of time. It, of course, records what time you actually charted your results so you could find yourself in quite a bit of doo-doo if your chart was reviewed and it was noted that you charted an 0400 assessment at 2300, but there was a way to do it. I didn't have my settings that way but I know of at least one colleague who did.
