Tracking nursing activities

My manager recently changed our DAR (daily activity report) format, such that now we have to break out our time with patients and identify whether we a) performed a skilled intervention, b) Observed/Assessed, c) provided teaching. She says Medicare is wanting to have a breakdown of nursing activities. Is anyone else being asked to do this?

This is a new billing requirement of Medicare, the agency must assign G-codes to each visit to indicate what type of service was provided. Be glad your agency is asking you to do this, a lot of places have just chosen one code to assign to every visit.

HMarieD is correct- this is a new Medicare requirement. What you should determine is what you spent the majority of your visit doing. Obviously, for most visits, you may be doing all three or a combination thereof.

We were told we had to choose one no matter what we did.

We also were told to choose one response, which ever was more evident with the visit.
