Here's a tip for the SATA questions. Think of them as each option is a potential answer true or false. Rough day so here is a simple example...
When assessing a patient for admission objective and subjective data is collected. From the following list select all the information collected that are considered to be objective.
a. Patient states pain is "8 out of 10" when moving, 4 out of 10 at rest"
B. patient's vital signs are T98.8, R 16, BP 120/80, HR 78
C. Patient presents with a crushing headache at right temple.
d. Patients skin is cool, pale, and moist to touch
E. Pupils are 6mm and react equally to,light and accommodation.
To answer: " is patients reported pain rating measurable/observable/objective" no
Are the vital signs given objective? Yes.
Is the description of the chief complaint observable/objective? No.
Is the description of the patient's skin an objective observation? Yes
Is pupillary reaction objective? Yes
So in this case the SATA answer would be : B, D, E
Does that help?

It helps alot. Thanks. But i have been using the nclexpn 3000 the sata questions are way more harder than the example you gave. All in thanks for the example.
Nclexpn study plan I only did Prioritizing and delegation questions out of the
Sanders CD I think it was 140,Infection control know what I would wear/what
they would wear,Lab values,I used Exam Cram did 648 questions,Incredibly Easypn
I did the whole coordinating Care,Maternal-Neonatal,and Child...I paid about $26
on the Kaplan did the Questions. You can get that at the your local library..
I also went to the library and got the Hurts Review which helped me out a whole lot on
Nclex..Good Luck
