Tips for sub folder?

Well, my doctor has pulled me from work (maternity leave). My last day is tuesday 12/10 when I'll be 39 weeks. I've been working on my sub folder but wanted to know what you all include in your sub folders/sub info.

I have

-a detailed list of all the kids who come for scheduled meds,

-info on my diabetics,

-med administration procedures (such as where all meds are kept, how the med cart is organized, giving PRNs, etc),

-documentation procedures (documenting student visits, phone calls, how to dismiss an ill student, etc)

-patient non-specific standing orders that the district NP has written

-first aid/accidents/illness instructions, written by the NP

-a list of students who I see a lot with chronic conditions

- a list of falculty/staff I work with a lot

-how to fill out student incident reports

-paper health office vist forms... we use SNAP but the sub won't have access. My aide will put in visits but she is not always here

-instructions on how to do work permits (the health aide also handles them usually)

-school emergency procedures

-map of school

-FAQ of subs

What else should I include? They will be having different subs in, and as of now I'm not sure if they'll have a long term one in our not. I'm due dec 17th and plan to be back mid-end of February.

I have lists, broken down by grade level and then teacher, of all the students with any health conditions. (These stay in the sub nurse folder, never given out to anyone.) I also have a separate one for food allergies and intolerances.

Label any of your drawers or files that might be confusing - I had a sub who couldn't find the pill counter. Now all my drawers have a list of what's inside.

Congrats on your baby!

Wish I could sub for you. Sounds like a well run and organized set up. Enjoy your family time off!

First thing first, **HUGS**! I am 34 weeks pregnant, and planning for my maternity leave as well. This thread is very helpful to me.

So far my sub folder contains:

-Contact list: includes names, phone numbers, and emails for important contacts such as myself, other RNs in the district, the district RN, etc.

-My daily schedule, which includes blood sugar checks, insulin administration, and medication administration.

-Simple instructions on use of our computer charting system.

-Information on Diabetics

-A list of students with medical conditions, which directs them to my binder of IHPS (health plans).

-County Office of Education School First Aid Guidelines.

I plan to do much more but I figure if I have to leave work earlier than planned that a sub would do fine with the information I have provided thus far.

I will be using this post as a reference as I finish preparing my sub binder!
