Agree with above, keep it short and sweet. If it was a supportive and nurturing environment, can't hurt to include that so the powers that be know that they have a good team.
I agree. You might be asked by your supervisor or HR to elaborate, but I would still keep it neutral. You never know who you might want to be on your side in the future.
"After ____ years with _____ hospital/facility, I find that I must submit my resignation. My last day to work will be ______. (Don't say effective of or give a pay period, just say when you will last work).
