ok, was given this little quiz at work. either i did'nt learn this in nursing school, or i have forgotten.:uhoh21: so here is the question. what are the three c's of documentation? also would like to see others responses to this. list six characteristics of good documentation...

date, time, correct chart, assessments, interventions, response, plans (recomendations), name and title, and a partridge in a pear tree :melody: :melody:
