Thank you very much for your reply. I really appreciate that you guys have showed me the best way to ask a question here. And I apologize to anyone that I may have offended. I am sorry.

Here I have re-posted the questions with my thinking and looking forward hearing you guys to give me some suggestions on my thinking. Thank you.

1)A patient with bipolar disorder, manic phase in the psychiatric unit states"I can leave anytime i want. why am i still here?". The most therapeutic response would be:

a. you sound frustrated and want to go home (i know in mental health you want to prompt the pt to explore his thinking behind what he said, so this one sounds right)

b. we would prefer that you were more settled before you leave (this one provides the pt with the fact in a polite way, so could be helpful for the pt as well)

3) A patient is right-handed. ECT would most likely be given by placing:

a. one electrode on the right side of her skull

b. one electrode on the left side of her skull

c. two electrode on the right side of her skull

d. two electrode on the left side of her skull

(this one i really have no idea)

4) A patient had a R) below knee amputation, to prevent flexion deformity of the limb, you should do all the following except:

a. give him a knee pillow to relax his quadriceps muscle

b. encourage him to lie prone three times/day

c. place him in a chair twice/day with his left leg extended

d. assist him to do straight leg raising

(i think it is A. because doing A will results the residue limb to be held in a flexed position, which will cause permanent flexion deformity. is my thinking right?)

5) A patient a laminectomy, which of the following would him to encouranged to do when allowed out of bed on the 2nd/3rd post-op day:

a. rest comfortably in an easy chair

b. sit on the side of the bed

c. stand erect

(i don't really know this one, but i think it is C, because doing C keeps the spine straight and also a and b are kind of similar)

6) which of the following is true in normal labour:

a. foetus is born at term

b. the foetus presents by vertex

c. the onset is spontaneous

d. all of the above

(i think it is d.)

7) (this question is specifically based in NZ) The registrar of births and deaths must be notified of birthplace, sex of child and parents address within:

a. 14 days by the occupier of the premises

b. 48 hours by the occupier of the premises

(what i found was both parents of child must notify births, deaths&marriage as soon as reasonably practicable after birth, usually within 2 months of birth. just the time frame is really different to what it provided in the choices)

8) If you palpate a woman's uterus 3 fingers above umbilicus and displaced to one side in the postpartum period, you should suspect:

a. a full bladder

b. she has retained placental fragments

(initially i was quite sure it is A. but then i read it somewhere b could cause this as well, that is why now i am confused)

found out the answer to this one by using google. it's on the left, but please look up the reasoning behind this.

I rarely post on here.

I know this sounds harsh, but you REALLY need to look up the reasoning behind your answers, or what u think r the answers. Study WHY your chosen answer/s would be correct, and do some further reading. You won't learn just getting answers off the internet (and they could be wrong, have u thought of that?)

If you don't understand one concept, you can't reason re other concepts/answers for other questions, and you will have many questions as a nurse. My brain is running all the time at work with questions about patients' conditions, why am I giving this drug/IV fluid, why does so & so want this exam/study/scan done?

I have done the hard study (still doing it) and the only way to learn is to do it yourself. You really are not doing yourself any favours asking on the internet. You will only get in trouble later and wonder why you didn't do the long hours of study.

Said with experience and empathy - I know what it is like - been there, done the hard yards!

(because i looked after a patient who had laminectomy, i remembered he was walking around in the ward 2nd/3rd day post-op, that is why i thought the answer could be c. plz correct my thinking if it is not right.)
