  1. I have found the answer in the book, thx everyone anyway~~
  2. Can anyone tells me a few situations that you would avoid using IV opioids on a pt?? I have thought about a couple like when a pt has allergic reactions to the opioid; when pt is confused..bt i just need a few more clinical situations...cnt think of any of others... Many thanks!!
  3. hey everyone, thanks a lot for your reply. you are right, it is really depends on the situation and how prepared the patient is. this was just a very vague question that i have been asked without any clinical backgrounds, and i think that was why i found it was hard to answer. as both of you have suggested, i will have a talk with the stomal therapist or stomal nurse in the hospital. Thx again!!
  4. Hi does anyone know following the formation of a colostomy, is the patient most likely to denial the existance of the colostomy (as denial is the first step in the grief process) or is the patient gonna reject relatives and friends (coz he thinks they won't like him anymore since he has got this poo bag attached to him)? Many thanks!!
  5. hi carolmaccas66: thank you very much for your suggestion. i will definitely bear your suggestions in mind in my future study. they are very helpful.
  6. hi ukstudent, thanks a lot for your help. i will definitely do some further reading into the answers to understand the reason behind it. the question that you would like to see the d answer is here. coz i think the d answer is not right, that is why i omitted it. 5) a patient a laminectomy, which of the following would him to encouranged to do when allowed out of bed on the 2nd/3rd post-op day: a. rest comfortably in an easy chair b. sit on the side of the bed c. stand erect d. move around freely in a wheelchair (because i looked after a patient who had laminectomy, i remembered he was walking around in the ward 2nd/3rd day post-op, that is why i thought the answer could be c. plz correct my thinking if it is not right.)
  7. Hi All: Thank you very much for your reply. I really appreciate that you guys have showed me the best way to ask a question here. And I apologize to anyone that I may have offended. I am sorry. Here I have re-posted the questions with my thinking and looking forward hearing you guys to give me some suggestions on my thinking. Thank you. 1)A patient with bipolar disorder, manic phase in the psychiatric unit states"I can leave anytime i want. why am i still here?". The most therapeutic response would be: a. you sound frustrated and want to go home (i know in mental health you want to prompt the pt to explore his thinking behind what he said, so this one sounds right) b. we would prefer that you were more settled before you leave (this one provides the pt with the fact in a polite way, so could be helpful for the pt as well) 3) A patient is right-handed. ECT would most likely be given by placing: a. one electrode on the right side of her skull b. one electrode on the left side of her skull c. two electrode on the right side of her skull d. two electrode on the left side of her skull (this one i really have no idea) 4) A patient had a R) below knee amputation, to prevent flexion deformity of the limb, you should do all the following except: a. give him a knee pillow to relax his quadriceps muscle b. encourage him to lie prone three times/day c. place him in a chair twice/day with his left leg extended d. assist him to do straight leg raising (i think it is A. because doing A will results the residue limb to be held in a flexed position, which will cause permanent flexion deformity. is my thinking right?) 5) A patient a laminectomy, which of the following would him to encouranged to do when allowed out of bed on the 2nd/3rd post-op day: a. rest comfortably in an easy chair b. sit on the side of the bed c. stand erect (i don't really know this one, but i think it is C, because doing C keeps the spine straight and also a and b are kind of similar) 6) which of the following is true in normal labour: a. foetus is born at term b. the foetus presents by vertex c. the onset is spontaneous d. all of the above (i think it is d.) 7) (this question is specifically based in NZ) The registrar of births and deaths must be notified of birthplace, sex of child and parents address within: a. 14 days by the occupier of the premises b. 48 hours by the occupier of the premises (what i found was both parents of child must notify births, deaths&marriage as soon as reasonably practicable after birth, usually within 2 months of birth. just the time frame is really different to what it provided in the choices) 8) If you palpate a woman's uterus 3 fingers above umbilicus and displaced to one side in the postpartum period, you should suspect: a. a full bladder b. she has retained placental fragments (initially i was quite sure it is A. but then i read it somewhere b could cause this as well, that is why now i am confused)

  9. hi all: thank you very much for your reply. i really appreciate that you guys have showed me the best way to ask a question here. and i apologize to anyone that i may have offended. please do not take it seriously, i do not mean that. to ceridwyn specifically: i did hit the kudos button for a quick "thank you" yesterday, i don't know why it was not shown. here, i thank you again for your help. and also, yes, there is a national exam for rn in nz, is called "state final". you can ask any nursing students, they should know it. it is 3 hours long and comprises 180 mcq. there are heaps of obstetric and mental health nursing questions in it. the questions that i have posted are the actual exam questions that are released by the nz nurisng council for sample questions.
  10. Dear Scrubby: I am not trying to get anyone to do my homework for me: 1) Mental health questions are very confusing 2) We have only be taught a little bit of maternity nursing, but are asked so many maternity questions in the state exam, which is ridiculous anyway. 3) I have tried my best and looked everywhere to find an answer, but could not find a definite answer for these questions. Please do not comment on someone without knowing that person and what that person has done.
  11. I am a nursing student who has got a few questions from my study booklet for the registration exam. I welcome any experienced nurses who can help me with any of the following questions. A big thank you to you all!!! They are all MCQ with only one correct answer. 1)A patient with bipolar disorder, manic phase in the psychiatric unit states"I can leave anytime i want. why am i still here?". The most therapeutic response would be: a. you sound frustrated and want to go home b. we would prefer that you were more settled before you leave 2) The most common side effects of chlorpromazine are: a. drowsiness, hypotension, restlessness, tremor b. photosensitivity, hypertension, tardive dyskinesia 3) A patient is right-handed. ECT would most likely be given by placing: a. one electrode on the right side of her skull b. one electrode on the left side of her skull c. two electrode on the right side of her skull d. two electrode on the left side of her skull 4) A patient had a R) below knee amputation, to prevent flexion deformity of the limb, you should do all the following except: a. give him a knee pillow to relax his quadriceps muscle b. encourage him to lie prone three times/day c. place him in a chair twice/day with his left leg extended d. assist him to do straight leg raising 5) A patient a laminectomy, which of the following would him to encouranged to do when allowed out of bed on the 2nd/3rd post-op day: a. rest comfortably in an easy chair b. sit on th side of the bed c. stand erect 6) which of the following is true in normal labour: a. foetus is born at term b. the foetus presents by vertex c. the onset is spontaneous d. all of the above 7) (this question is specifically based in NZ) The registrar of births and deaths must be notified of birthplace, sex of child and parents address within: a. 14 days by the occupier of the premises b. 48 hours by the occupier of the premises 8) If you palpate a woman's uterus 3 fingers above umbilicus and displaced to one side in the postpartum period, you should suspect: a. a full bladder b. she has retained placental fragments
  12. A RN, who is not familiar with unit policies and procedures approaches a new graduate nurse and requests her to access a patient's central line. The new graduate nurse has not yet achieved competence in this clinical skill. What is the best response of the new graduate nurse or what is her best action under this situation? Thank you very much for anyone who can help me~~
  13. no lakflem, i am talking about the transition program for nurses who graduated from a bachelor of nursing course.
  14. hi, i have got two situations from my clinical course that i could not find the best action towards them. i am hoping anyone who have been in those situations or know how to deal with them can give me a hint. thank you! 1) what would you do if you were on a night shift and two nurses phoned in sick at the last minute? 2) when you are confronted by a patient or family member, how you would handle it?
