SMART goal for family education

I need help submitting SMART goal in community setting focusing diet, exercise and emotional wellbeing.

It means i have to make teaching plan in SMART goal.

Please help

Is this for an actual patient or a scenario that you have been given? What thoughts do you have so far? There is no details given in which one could base a nursing diagnosis or goals upon. All nursing diagnosis and goals need to be based upon what the patient needs, what I may say for a 90 year old woman when it comes to diet is going to be completely different than what I would recommend or see as the needs for a woman in her 20's that is pregnant. Their needs are different and so will be the goals for each individual.

It is community health nursing. Family [COLOR=#000000]plan that includes S.M.A.R.T. (Specific, Measurable, Achievable, Realistic, Timebound) objectives, follows teaching/learning principles; incorporates the three domains of learning; and is justified by relevant literature. Nuclear family with one year old daughter. New move to Canada, diabetic mother and very little support resources. I am really struggling as family based goal seems little complicated. I would focus on diet, physical activity and emotional support.

The family consists of a mother (38) and father (40) and their biological daughter SV. The family lives in Shelburne Ontario. The mother is asian 38 years old and had worked as a travel agent before birth of her daughter. She is staying home mom at present. This family recently immigrated to Canada for better career opportunities. Father was in IT backhome and now working as a production worker in a company. Both parents are university graduates and are still in transition phase because of recent move to new country and new parental role. This family has extended family back home and have a sister as a support group who lives close to them. Mother is diabetic and her blood glucose level is above normal range due to caregiver stress, less physical activity and new mother's role.

S.M.A.R.T Goal: to improve physical activitySpecific: Workout 3 days a week for half an hour or more.Measurable: Time walks, track number of steps taken and keep a daily log. Keep a daily log of calorie intake and daily weightAttainable: Will lose 4 pounds in two weeks by exercising regularly and eating healthy.Realistic: Will start with walking 10 minutes a day gradually increasing to 30 minutes.Time based: By July 15 will lose 4 pounds.

Ever had a baby? LOL, plans rarely work out when you're a new mom. If somebody told me to keep a log of anything when my first was new, I'd have laughed, if I wasn't crying first. Nowadays at least she could get an activity tracker wristband or something.

What does this family tell you when you assess them? What is important to them? What do you know about Maslow's Hierarchy of Needs, and how would that relate?
