Short term disability while in IPN/rehab

Does anyone use short term disability through their insurance while being at rehab? I am finding difficulty finding out which physician is the one should be filling out the forms? Is it the evaluator, the doctor at rehab or primary care physician? Would appreciate any answers. Thank you

When I was in a program my Short Term Disabilty papers were signed by my psychiatrist who I was required to see on a regular basis for medication management. I got my disabilty through the state (not my insurance) A program I had paid into under the diagnosis of Major Depressive Disorder because in CA the state disability excluded Substance Use Disorders. I was not gaming the system as I certainly was depressed.

In CA, the paperwork can be completed by numerous sources. I've heard of some folks even getting a chiropractor to complete it. Not sure what state you're in, but in CA, there are numerous options.
