Alert and Independent (as far as ADLs go)?

Alert & Intact (as in neuro intact), maybe?
Definitely not a type, I work at two facilities and they both use it on report. It's hand written on the 24 hour report for multiple residents. I'll just have to ask. I can't figure it out
That was my best guess to because falls (esp when head is hit ) are usually on neural checks so...who knows !! Lol

I know this is one of those things that is so simple but I just can't figure it out ! What does "A & I " stand for. I always see it on report and I'm embarrassed to ask I feel so stupid. I know it has to be something like A&O...please help!! I've never used this abbreviation before but need to figure it out asap.
