Roster/Sample Matrix - who keeps it up to date at your facility???

Hi, In my facility my DON is managing that, In my old facility before my MDS days I as senior charge nurse on my unit kept track of it until they added Unit Managers... Hope this helps..

MDS takes care of it. The DON is clueless. When we are expecting the state surveyors, it is done every morning. Our MDS comes in at 4:30 am so that she can update with ABTs, falls, etc. Unit managers help to some extent but the MDS coordinator basically does it.

The only facility where i worked where it was done right, the MDS nurse did it. The unit managers really don't keep up with who has a foley and who has a wound. They think they do, but they don't.
We also did it every morning during survey window.
And, I wonder, WHY don't we use it daily or at least weekly to give ourselves a bit of a heads up on what's actually going on? Why doesn't management realize it would be a great staffing tool? Most DON's end up being firefighters for the fire du jour, and can't step back long enough to get a handle on the big picture.

Hi in florida here we don't have to do the matrix any more we do the short form think it is the 672. Our DON tried to keep up the matrix but we also used the floor nurse and I am the restorative nurse so alot of it I was trying to keep up with. We are being surveyed now with the new QIs survey it is very different

I am a lucky nurse. My software takes from the MDS into the roster from the most recent MDS.. on the face sheet I can adapt who is admitted with contracture, or foleys, or if someone is on an ABT. Also where I put the flu shot & the pneumovac. When I update the care plans (as needed every day) I review this area as well. The better the computer program The better things work.
very nice program --- it is probably expensive -- my facility cooperates believe this philosopy: Well we will just pay the fines when the state sends them instead of making sure we dont get them. dang it is fustrating!!! Got yelled at today for coding in the mds that a flu shot was not given well hello we did not have it-- I can not give what I dont have. and there is no comment section to say our pharmacy says we are out!!
our program with the MDS also has the ability to pull out the stuff for the Matrix but you had to manually put in the toilet schedule, flu and pneumo vac. and a couple of other spots. We just went through the survey the last week of Oct. With the new QIs survey we done't need to do the matrix. I do know your furstration I used to help the DON fill out the matrix now I do most of the 762
