Hi its been awhile since I have been on so hi to everyone. Have a question for everyone out there has anyone started the cultural change? We are trying to get started at our facility adn it is very slow in coming. Anyone have any suggestions or have tried something that has worked? I know the mind set for all staff needs to change first and that everyone from Administration on down has to be involved and on board. So please help if nayone has any ideas or have some success stories please share. Thanks from florida

Hi sitcom its debbie from florida how are you doing. I have a quick question I know in restorative there is no such thing as a quick question. Our administration is asking for a manuel for the units. Now this isn't just a restorative manuel but it is so that all shifts at all times knows what to do. I guess you would call it a combination restorative/activities. We are about to reopen our new solariums/day rooms and they want something to be doing at all times. Any suggestions? We have come up with putting in the walk to dine or wheels for meals. Then toileting/grooming after meals but looking for a better name. We have named a few games ball toss trivia. They kinda want it so everyone knows what they are to do and at what time. unrealalistic maybe but need to put the best effort. Any ideas or suggestions from anyone would be appreciated thanks for listening.

Roster/Sample Matrix
our program with the MDS also has the ability to pull out the stuff for the Matrix but you had to manually put in the toilet schedule, flu and pneumo vac. and a couple of other spots. We just went through the survey the last week of Oct. With the new QIs survey we done't need to do the matrix. I do know your furstration I used to help the DON fill out the matrix now I do most of the 762

I just read your last note over. The part with distance I put it on the CNAs sheet they sign daily it is walk to dine 10-20 feet or others is longer and either 2 or 3 times a week 5-10 minutes each and that gets my 15 minutes of ambulation a day.Since the MDS states it allows the time for set up and getting the task done.

Hi I know what you mean about the new survey. We had ours the last week of October came in on Monday and left friday about 5. They really didn't get into restorative at all. We got a cite for a PT order not done when someone was admitted the supervisor marked it on the POS but didn't send notice to therapy. We also received one for giving meds in the dining room they said a resident who feeds self independently was given meds on a spoon. I felt that they really couldn't find anything so they went digging. Was a very stressfull week put in at least 10 hour days. Still working on getting a great restorative program up and going. Am in the process of taking residents off restorative ambulation and place on walk to dine with floor staff to free up restorative staff for other programs. We still have some that are kept with restorative ambulation. Is your restorative note a narrative? What types of assessments do you do quartely and do you do them on all your residents. we do a fall assessment and a narrative note trying to find one that is more of a check list formate. Has anyone started working with the WI with your residents if so how is it going? Well thanks for keeping this thread going it is great to be able to talk to others in the same situation as I am in.

Hi in florida here we don't have to do the matrix any more we do the short form think it is the 672. Our DON tried to keep up the matrix but we also used the floor nurse and I am the restorative nurse so alot of it I was trying to keep up with. We are being surveyed now with the new QIs survey it is very different

Hi I know what you mean about the RNAs being pulled and restorative being responsible for everything. We get pulled to work the floors, find missing wheel chair parts you know how leg rest walk away during the night . How do you keep track of alarms and keep them from being broken? We are excited we just had our solarions remodeled and now we are getting ready to start different programs. You say you have about 60 programs going boy do we need to talk I need so much help. What types of programs are you doing? Do you have group programs , one on one. How do you do it? Do you do the care plans and MDs? This week we are being surveyed so next week I have to start working very hard in getting things started. Do you chart on all the residnets or just ones in restorative? yes we must keep this thread going and keep in touch my goal is to have the best programs and keep the staff positive and have some fun with both residents and staff. Too many times the resident say how bored they are. I want to keep them at their highest functioning possible. Thanks for any input you have

Hi I am a LPN working as a restorative nurse in Florida. I am starting up our restorative programs. Ambulation we put it on the CNAs flow sheet that the resident is walked to dine which is a constant battle but, then we also have residents in a restorative ambulation which is done by RNAs. We are also working towards cultural change in the facility and trying to get things going with activities and cnas for ROM/ exercise groups and more restorative type groups. we can do restorative programs with a 1-4 ratio. Trying to get the floor cnas invovled with working with the residents in the day rooms are hard and will need alot of re-educating. We are also trying to get the residents invovled with working with the WI which should be fun. Would love to hear more about your programs and how you document. Do you do the MDS if so which parts? ok guess I really wasn't thinking or putting th two together I am in the colege network they offer a 4 hour tutoring class one day a week thanks alot I have already paid for the first part that is why I have to pass these test but is it enough to pass the boards or clinicals?

I too am looking for a certified restorative nursing program and haven't found one so if anyone knows of one please let me know. Am working as a restorative nurse and in need to get programs started like yesterday. I am also looking for a quick and easy restorative assessment for quarterly, admission,sig change that may use a check system with a summary any advice would be appreciated. Thanks

dementia specific unit
Hi my name is Debbie and I am a LPN and work in long term SNF. We are trying to start some dementia specific programs in the facility. I'm in charge of getting it started is there anyone out there with any knowledge of any programs or have any pointers that would help me with this project.
