I have a patient who had a gait dysfunction and has fallen twice within the past 3 months. My nursing diagnosis is:
Risk for injury r/t fall risk.
I just don't know what to include as short term and long term goals other than the patient will remain free of injury. Any help is greatly appreciated!

Other goals can include
-Patient (or even patient's family/caregivers) will express an understanding of ways to prevent falls
-Patient will demonstrate appropriate use of assistive devices (cane, walker, grab bars)
I always find that that goals pertaining to educating the patient and family make a care plan come full circle. If you don't do something that can help change a behavior you are probably going to see the same patient, with the same problem time and time again.

not to be insolent here, but what other information do you have about the patient because there are reasons that this person fell and they need to be identified. the gait dysfunction, for one, should be included as a risk factor in the diagnosis as well as a history of falling. the taxonomy has a long list of risk factors for this diagnosis. you need to look at them and include any other as r/t items and address them in your interventions. see risk for falls. goals are the predicted results of your nursing interventions. what are the interventions you are planning? one long term goal is for the patient not to fall. remaining free of injury is more appropriate for risk for injury.
