Returning to work from psych hospitalization

I just spent a week in the psych ward and am still in the partial hospitalization program right now. I'm going through with filing for short-term disability but ultimately I'd like to go back to work soon. I'm thinking of transitioning back to work when I've been given more of an "okay" and am feeling better by doing a half-day PHP and then half-day at work, or the opposite way, depending on the work day. Has anyone done this before? It's been a while since I've been employed during a psych hospitalization - the first time was in 2010 when I was a psych nurse and the last time was 2012 when I was a health science teacher, on my "spring break". Suggestions and feedback on balancing work and self-care would be greatly appreciated.

[FWIW, it was my boss that was a trigger for my hospital admission. I will be having a meeting with her no matter what, before I return to work in my capacity. It wasn't so much anything she did, but her personality and behaviors towards me. I felt micromanaged and it really increased my anxiety. I'm also taking 6 credits in graduate school while working full-time. I'm reconsidering my school enrollment right now.]

My opinion is that you, as a minimum, should move away from this individual in the workplace since you say she was the trigger. If you need to find another job, maybe that will be the answer. Do not think it is necessary to stress yourself in this manner if you want to be successful with as little problem as necessary. Just how I see it.

Have you thought about working part-time or job-sharing? I know it's difficult in your position, but if you could cut back to, say 3 days/week, you might do much better and still have time for a life outside of work. That may enable you to better tolerate your supervisor, as you won't have to deal with her 5 days a week. Also, you might consider working weekends when the boss isn't there.
