Requirements for the board exam- nle 2011 (philippines only)

1. [color=#ef0462]original and photocopy of transcript of records (tor) with scanned picture and with remarks "for board exam purpose"

2. [color=#ef0462]original and photocopy of birth certificate from the nationalstatistics office in nso security paper. those whose nso copy are notreadable must submit a birth certificate issued by the local registrarof his/her place of birth

3. married female applicants must also submit marriage certificate issued by the national statistics office innso security paper

4. [color=#ef0462]original related learning experience duly notarized

5. [color=#ef0462]original or-dr cases duly notarized

6. current community tax certificate

7. [color=#ef0462]four (4) pieces of passport size pictures in white background with complete nametag

8. [color=#ef0462]1 piece window mailing envelope with metered postage stamp

**** for those who haven't filed yet. you can also file your application @ prc ~ extension service center at ever gotesco manila plaza, recto avenue, manila. (located at the 2nd floor of ever gotesco u-belt.) masmaganda kasi konti lang ang nakakaalam dito and take note naka airconditioner pa kayo hindi gaya sa main mainit.. hehe

1. they [color=#993300]open @ 9am but if you plan to appear there earlier much better so you could fall in line first. (before, i went there at around 8:00 am ~ kaso may mga nakapila na mga 15 persons ganun, basta agahan nyo nalang)

2. secure a [color=#333399]blue form (kung minsan nauubusan sila kaya ordinary bond paper nalang) saka may isa pa pala atang form na i-fifill up (not sure na). after filling up the forms ~ you are going to pay for the [color=#993300]stamps (3) ata + window mailing envelop. worth p80.00 (pero baka nagmahal na sila ngaun)

3. then, fall in line for the passing of requirements. when they finished checking your requirements [color=#993300]proceed sa cashier then pay the necessary amount. p900.00 samin last time.

4. keep your [color=#333399]noa (the blue form). you are going to bring it during your 2 day examinations.
