OK...I can't find my current license anywhere.

I've found the PREVIOUS one. Only thing I can think of is I shredded the new one when cleaning out some files, thinking it was the old one (thwapping self upside the head) :selfbonk:

Went to the Texas Board site, got the letter written, check made out, envy addressed.

The Question: Do I need to have my signature notarized before sending it in? The FAQ on the site does not say so, but somewhere in the back of my mind I recall coming across something like that a couple of years ago when I was looking for something else there. (my DON as insisting that a LVN could "take an order to pronounce" I knew that was wrong, and wanted proof. anyway, I digress)

Does the website list a phone number? Might want to give them a call. They may even be able to complete the transaction over the phone. I could see them asking for a signature to be notarized if you were making any changes, but if you're only asking for a replacement copy and they're sending it to you at your address, seems like that shouldn't need verification.

TX board is notoriously difficult to reach by phone. Extremely long waits/phone tree to go through.

I think I'm gonna run to the bank and get it notarized before I send it off. It won't cost me anything, won't hurt anything to have it notarized, and if it MUST be notarized I won't have to wait for the turn around time for them to send it back to me.

Hi - looked this up for you! From the Texas Board of Nurse Examiners -

Question: I lost my license. How do I get another one? Is there a fee? How long will it take?

When a license is lost or stolen, you should report it to the BNE as soon as possible by doing the following:

Send us a letter with your name, address, license and/or social security number;

Provide a statement as to the disposition of the license;

Attach a $20.00 Check or Money Order;

Mail the $20 fee and statement to:

Licensing Department

Board of Nurse Examiners

333 Guadalupe, Suite 3-460

Austin, Texas 78701

Upon receipt of the fee and statement, we will mail the replacement or duplicate license within ten working days.

Thanks a bunch Ashera--that's just what I did. Went in the mail Thursday.

THere's just something stuck in my head from a couple of years ago that says it's supposed to be notarized, but the current info doean't say anything about it.

I guess I'll find out in 10 days or so
