I'm confused. can someone help me? I received my white postcard last week and I can't send it since i still don't have my candidate ID number. I have no other options but to register by mail since I don't have any credit or debit cards. My problem is when i received my white postcard from Vermont, there was no registration form for me to fill up and the forms can't be found either on the Pearson or NCSBN websites.
Where can I find the registration form for NCLEX? Help me please...

actually there are 3 ways to register at pearson vue either by mail, phone, or online. if you chose to register by mail you will be needing a us bankdrfat check drawn at any banks payable to pearson vue . although this route of registration may take weeks or months depends on the courier service before you recieve your candidate number given by pearson vue.
your question is there is no form sent to you from vermont?
why not try to email the vermont bon , because in my case when i received my eligibility kit from my bon there is an candidate buletin which has a mailing envelope with application form inside it for those who want to register by mail.
however, if you register either by phone or in online you will be needing credit card on hand for the mode of payment. if you dnt have any credit card why not try to borrow others credit card. usually mostly of those who register at pearson vue chose this route since it is simple and quick way to recieve your candidate number and your ATT at your email address .

:chuckle yeah, i didn't receive anything except for my white postcard and a letter of instructions for registration online. ughh... i actually e-mailed them about this (and also e-mailed pearson/ncsbn). this is what the vermont bon said to me...
[color=maroon][color=maroon]"you register with pearson vue online, they have a registration form that you do from their website. if you do not want to register with them online, you can request a paper copy of the registration packet (it is faster to register with them online). all of the instructions are in the letter that was sent with the card. you should go to the pearson vue website to find out more about registering by mail."

Thanks for the info... Hihihi... I was actually scouring in the internet for the forms, but couldn't find any. All sites kept pointing me to the Candidate Bulletin which isn't really providing me with my reg form...
I'm planning to take it in Makati. Ikaw? :chuckle I just emailed them requesting for the reg packet.
i guess every states provide a candidate bulletin handout. i wonder why vermont bon did not send you that. soo youll be taking it in makati.
anyway im done with nclex.-rn. i took it too at trident tower in makati.

hi!! i have a related concern.. i received my white card from Vermont which i'm supposed to return with the candidate number. My concern is, will i have to fill ALL (as in ALL--back and front!) of the blanks with my contact informations? Please help! thanks so mush!
