Refused only 1 med but they're already all crushed together?

In LTC (I don't know if this is wrong to do..) a lot of times residents will have a 20 pills in the AM and need them crushed.... So if you get them all crushed and in apple sauce and present it to them and they refuse one of the meds in the mixture but want the other 19.... what do you do? Oh and how often does this happen?

lol too bad we are in a world of "cover your *** medicine" meaning MDs treat every single diagnosis...I argued w/ one just the other day over why are we giving this tiny 90yr old fem (hoyer only) contracted, not a fall rish as she really can't move anything fosamax. Not to mention it's not crushable and she's a crush/ thickened liquids....and he says "oh but she's high risk for a stress fx".....the man also has her on every vit. and calcium w/ D lol. Some MDs are more reasonable than others. In terms of the massive amt of psych meds we give. I feel our resident's quality of life would be most improved by more activities/ outings and exercise...so much of their discontent I think stems from boredom and loneliness.
