Hi, I am in desperate need of advice. My client is a 5 yr old with many medical issues. Our current problem is that she is on total parenteral nutrition. She gets probiotics and prune juice when needed. Her stools are always liquid and she has frequent smears. Her bottom is absolutely raw. We use pink av diaper rash ointment and protective cream however it is not working. I keep her open to air as much as possible and encourage the other nurses and the parents to do so as well. I have also read that using liquid anti acid on the affected area can help so I am currently trying that. Does anyone else know of other tips or remedies that might help? I am posting this with moms permission. Any advice is greatly appreciated.

A&D mixed with diaper cream. It's a simple. It classic trick bc the diaper cream is so thick it is often counter-productive

I'd make sure you aren't fighting a yeast situation. If that's the case, all creams are futile until you get some nystatin on it!

I used to take care of a chronic kiddo with a similar situation.... we would tuck an 02 tubing into his diaper at night while he slept. ;) Being inpatient has some perks. ;) LOL!

I recently learnt about using stoma-adhesive powder mixed with Desitin to apply on the excoriated areas. We also use Cavilon spray.

Not sure if you have access to O2 but it works beautifully on a red bum! Have to have parents or someone supervise, this works depending on age.

We use Sensicare (Red cap), it's super thick & so moisturizing. It literally will cling to her bottom and help a lot. We have had patients with terrible diaper rash and it tends to work really well.
