My facility would like to move away from ss insulin. I used to work at a hospital where the MD would write orders for range dose insulin. I cannot remember the ranges though. Can anyone help me?

200-250: Give 2 units Novalog
250-300: Give 3 units NOvalog
300-350: Give 4 units Novalog
350-400: Give 5 units Novalog
For BS > 400: Call MD
Where I work, this IS a sliding scale. I forgot the term for using the equation, but it was something different.
Either way, the equation is pretty much the same thing....each blood sugar level will end up in a range (in our med rec system, you can type in the equation, BS-xxx/xx, and it will give you a sliding scale like you posted above). So, it's really not that different of a concept....
We have some docs that use a standard sliding scale, it's broken down in to high, medium, or low dose. The ranges are the same, starting with 121-150 and going up in 30 unit increments, but with the high dose SS you'd give 3 units for the first range, with the low dose you give 1 unit. We most commonly use the standard sliding scales for patients on TPN, rather than with brittle diabetics, since their needs are much more specific.
OMG!!! I gave a patient too much insulin!!!! I didn't realize what I had done until 2 days after. His BS was 207 (on tube feeding). Calculation was BS-100/10. I gave 10 units of Regular. I know I did something horrible.
Mistakes happen; you have to forgive yourself.
I do wonder, though, do you have a policy for having another nurse double check the calculation and dosage?
