Hey guys! I saw an old thread of this pop up recently and thought it was a great idea especially with a lot of us sitting for the NCLEX soon. I thought it would be good to start a new one where the old one was fairly long/old.

What you do is provide 5 interesting facts or things we ought to remember. Please make sure that your facts are correct before submitting and if someone spots a mistake that someone else has made, please be respectful in your correction. ;) Lets get started!

1) When determining acid-base disorders remember the acronym ROME

Respiratory

Opposite

Metabolic

Equal

2) When drawing an ABG remember to first perform the Allen's test to assess collateral circulation.

3) If a patient is in cardiogenic shock with pulmonary edema position them in high Fowler's position with legs down in order to decrease further venous return to the left ventricle.

4) When bowel obstruction are high in the small intestine (where gastric acid is secreted) blood gas will be alkalotic. When bowel obstructions are in the lower bowel (where the base solution is secreted) blood will be acidic.

5) To test for hypocalcemia look for Chvostek and Trousseau sign.

Chvostek is the constriction of facial muscles with gental taps over the facial nerve in front of the ear.

Trousseau is observed carpal spasms when a blood pressure cuff is inflated above the systolic blood pressure for a few minutes.

If you suspect an air embolism, lower the head of the bed and place the client on the left side... it traps the air.

1-After amputation phantom limb pain is temporary condition in this case u assured the client that the condition is normal and only temporary.

2-denture should be clean with luke warm water.

3-always change the water before perineal care to reduce the risk od UTI.

Ifosfamide - can cause ifosfamide induced encephalopathy -give methylene blue

- can also cause hemorrhagic cystitis so always give MESNA given PO

Chlorambucil - can also cause hemorrhagic cystitis - empty pt. bladder to prevent and can give MESNA too

Cisplatin - needs vigorous hydration

- #1 drug causing N&V -premedicate with 5HT3 meds

- cause severe nephrotoxicity - amifostine may be used as a renal protectant

-hold the drug if SCr is >1.5mg/dl
