"I" careplans

Sorry, but what is an "I" careplan, I thought they should be patient-oriented and not I oriented.

I have been using "I" Care Plans for the last four years in my facility. We all have, we own 7 homes in the Southern Illinois region. I like it. It is as if it is in the first person and more person centered, as opposed to the regular "medical" side. It is as if they are telling their story to staff and their families reading the care plan. So this is not new to me at all. I have trained multiple incoming MDS Coordinators using the I plan. It is more of a pioneering approach. Surveyors loved it when I incorporated the CP's four years ago.

Would you be willing to send me a sample through the message center here, or I could send you my email address? My DON really doesn't like the I format, but the rest of the team does, so I'd like to see how they should be done, and maybe start doing it that way.

I've been writing "I" care plans for a few years now. I like an individualized care plan as opposed to a generic, canned care plan and to me personally, the "I" care plans are more descriptive of the individual patient/resident/elder's needs and problems. They paint a better picture IOW.
