If you have a resident who was admitted and then discharged 4 days later, before the Admission assessment was done, do you have to complete an admission assessment after the discharge? Thank you

No, you don't do an initial assessment. 
If traditional Medicare A you would do a combo 5day/ discharge return anticipated or not anticipated, with an end of stay. 
For managed care you would need a5day do not submit, and then a d/c assessment. 
im in Ohio, IDK if your rules are the same.
