Unbeleivable!!
Yesterday we get a faxed report for a potential client. Basic info missing so I called to ask: what level ADL's. PT care, any decubiti, rashes etc.
Doc reassures me- light asst in ADL, no decubiti, no rashes, wonderful patient and family . So I say we accept him, Lo and Behold= Decubiti on the buttocks, has Scabies, totaly disoriented, full time care in all ADL, out of it, etc.
So I call the hospital and speak to the Doc who says "Come On, everyone here does it ,.I dont need to be better than the average!"
I told him that I hope he learns to strive for better than the average!!
He then tried saying I yelled at him and he will report me!!!!!
Can't win (don't worry my staff knows enuf not to believe him!!!
Well, my place has been taking morbidly obese patients recently. I'm talking way over 400 pounds. Problem is, we don't have the equipment for them! OK, we get a larger bed but what about lifts? And shower chairs? And they won't fit into the whirlpool......Sure enough, one ended up on the floor and had to get 911 to get them back into bed. Big incident report waiting to happen! Is this happening anywhere else?
We had a woman who was over 350 pounds so we rented a bariatric bed at a huge cost. She flat out refused to use it. Now she's back again at a whopping 444 pounds and is still refusing a larger bed. Our beds hold 450 pounds so we are all hoping that some of that weight is fluid.
Do any of you get arguments from the discharge planners at the hospital?? Their favorite line for me is "But you HAVE to take them...they live in town" Living in town does not get them in the door! I said no to a referral today. Alcoholic, drugs, and he smokes. Thank goodness my administrator always agrees with my clinical assessment to say no even though the census is down.

Pleased to report that because of this incident we were given budget go ahead to go check all potential new admits in person- so the above wont happen again and we will be able to ensure proper placement based on pt need.
