Is this for homework?

In only the last 10 years or so, there has been an increased emphasis on patient safety.

We have had workshops in communication and teamwork with emphasis on proper patient handoffs to reduce errors. There is a culture of less punishment for errors, and more focus in trying to find the root cause for errors and designing systems to reduce the chance of errors occuring.

Now we have checklists, timeout before procedures to make sure we have the right patient, right site, etc.

Computerized IV pumps with programming for different drips, all double checked with another nurse.

The pharmacy makes our drips. (No more calculating how much saline to withdraw from the IV bag and how much Morphine 15mg/1ml to add to make the right concentration)

No more bottles of concentrated potassium or sodium chloride(used in dialysate bags)sitting around the unit.
