If she said she had given it I too would sign given by AT/per KT and comment on the chart.

When signing off meds on the eMAR, there is a spot for "performed by." It defauts to whomever is signed on, however you can change it. You could put in "Smith, Jennifer" (or was it Katie?). I would do this and then also write a note about it "50 mg Metoprolol given by Jennifer Smith, RN per her report and the report of the pt." Needless to say, I would be unlikely to do this for a narc or insulin....
