Hi, I was wondering what do you use for Mental Status Questions for Neurology Nursing. Is there a universal questionaire?

What's your name?
when were you born?
what's the name of this hospital?
what month of the year is it?
who's the president?
what did you eat for breakfast?
what did i tell you my name was?
what were the three things i asked you to remember when i came in the room?
I don't know if its universal, but that's the practice where i work. sometimes all those questions, other times just one or two. sometimes just conversing with the patient regarding their care and treatment answers any questions about congnition.

what are you kids names (if i know them lol)

what is this place we are in
do you know why you are here
basically you want to ask questions that determine if the pt is oriented to person, place, time and situation. A&O x4 means they are oriented to each one.
