Willow, unsure what you are referring to. Are you saying the 'frequency' on the care plan is being changed? Or are actual dates on documents being changed? A frequency change on the POC or 485 can be made in the form of a MD order - it would read 'change to plan of care - SN 1 W 9 or whatever. Pls explain.

No I originally did two routine visits and they want me to change the 2 visits from a routine one to a discharge then SOC because the insurance changed and they did not know it. This happened 12/29/2010 and 01/01/2011 and they are asking me to change the visit types now.

As far as I know, it's technically okay as long as you made visits on those dates and collected the information required. You would need to write an addendum note to explain the circumstances of the situation and why the documentation was changed from regular skilled visit to OASIS data collection visits.

I have had to do this in the past due to insurance changes and it is okay. Reason for D/C is insurance change. You can indicate in the summary for the Admit that it was related to an insurance change as well

Thanks I was just not sure and I don't want to end up on the end of a legal mess.

The legal mess would be creating a visit note or OASIS when no visit was made.
