Question concerning unemployment benefit!!!!! helpppp

i was working for an agency which sent me to a rehab facility for 1 week. on my 3 days, i worked doubles and did a great job on my fourth day, i made a med error. i gave a patient who was not receiving ambien, ambien. it was my mistake which i shouldnt have made and that is something i hope never happens again. (but the facility has some blame to that i was left on an acute rehab floor with forty patients to pass meds and assess them) she did not take it, rather, she took it to the nurse manager. the nurse manager for the facility where i was sent made me sign some written warning. the agency said they were going to pull me out and i can come back and start work for their facility (the agency owned a facility). i called when i got back but the agency kept beating around the bush with me and said march was full, they will put me in for april. till today, they never told me i was fired or not.

i am trying to file unemployment and now i stated i was not given any more assignments for reason why i am unemployed. when unemployment verified that with the agency, they stated i was let go. i have to complete a form stating exactly what happened that i was let go but the problem was that nobody told me i was fired or let go. so am i supposed to fill in/write what i feel is a reason i was let go?

can unemployment report a med error to the state/nursing licensing department?

is anything i am doing right now with unemployment and this case going to damage my license because if it would, i will just forget it

Nobody at the unemployment office is going to notify the nursing board about you. They are too busy doing their own jobs. On the paperwork, you just tell the truth. Write down that you were given no termination notice in writing or verbally by the employer; the first time you heard about it was from the unemployment department. Expect any and all untruths from the employer to the unemployment office. They don't want to pay out your claim. Just counter with the truth, when you are questioned. That is all you can do. I would definitely write on the paper that the agency pulled you from the assignment, you did not quit it, nor were you sent packing by the facility. Write down the name of the person at the agency who stated that you were going to be placed in their facility and state that you were waiting for a start date but were not given one. This is the information that will get your unemployment claim approved, not the lie(s) told to them by the employer.

Were you paid via W2?

no i wasnt paid via a w2 form, but i would have to verify that. but what i dont understand is that the day i got written up, and the incident was reported to the agency, they asked me if i wanted to work as an aide and the agency stated coming back to the city, i will work in thier facility?
