    just called NMC regarding the resubmission issue and they said that they need only the blank form of the whole document because they need the bar code.. thank God. no need to submit the filled up...

    the document they received was the verification of license form, the form which you have to download and send it to them thru mail. it was the only form that says "document received resubmission"....
