    Noooo this is not the norm. These are things that need to be reported and stopped. Do not get into these horrible habits or you will end up harming a patient or yourself. Don't be one of those nurses...

    ok im just writing this out for my own sake b/c the simpler the better for me lol. increased INR = Increased Bleeding (longer to clot) Decrease INR = Increased

    Thank you for the input. the open area is just the epidermis so it really isnt deep at all. so could i say... open area will regain integrity of skin surfaces... I am having trouble with the outcome...
