    It doesn't matter which one you take as long as both are accepted wherever you are applying. Some boards in the States only accept toefl, so you need to check that out.

    Yes it can be a different hospital. And yes normally you"owe" them a couple of years. Depends on the hospital what they want, but around 2 years I think would be reasonable.
