   I think you need to repeat the process again.. and make sure when you send it the address is from DFA including the IDV form in it in a sign sealed envelope or else they won't accept

   Yes, they required that.. u need to get the form from them.. then sign and fill up bfore u submit to DFA phil together with your diploma from school and ched CAV.. then after that u mail it to BRN...
