For recommendations, I would choose a candidate who is likely to give you a strong reference letter. I choose my Supervisor at my current job and two professors. Hope that helps.

I don't believe everyone is doing it. I think the people who are doing are just trying to explain any hiccups or temporary school leave or anything that might have affected them in undergrad.
